L-Carnitine vs Acetyl-L-Carnitine: Which Reaches the Brain?
L-carnitine vs acetyl-L-carnitine is mostly a question of which form was studied. The memory research we cite used acetyl-L-carnitine at 1.5 to 3 g a day. Synaptril lists plain L-carnitine, with no printed amount, so those studies do not match the capsule.

Is acetyl-L-carnitine the same as L-carnitine?
No. Acetyl-L-carnitine is a related form of L-carnitine, and the two are sold and studied separately.
The Synaptril label says "L-Carnitine", with no "acetyl" in the name. If a product used the acetyl form, the label would have to say so.
Which form did the memory studies use?
The cognition research used acetyl-L-carnitine at about 1.5 to 3 g a day (NIH ODS, Carnitine). A Cochrane review of 16 trials covered that form only and found no evidence to recommend routine use (Cochrane, Acetyl-L-carnitine for dementia).

Gram doses against a ceiling of 58 mg is a gap of roughly 25 to 50 times.
What did the acetyl-L-carnitine trials find?
The trials found small, short-lived gains and no lasting benefit. The Cochrane review pooled 16 double-blind trials in people with mild to moderate dementia or cognitive decline (Cochrane, Acetyl-L-carnitine for dementia).
Clinical global impression favored acetyl-L-carnitine at 12 and 24 weeks but not at 52 weeks. The MMSE score rose by 0.69 points at 24 weeks only, with no gain in dementia severity or daily function (Cochrane, Acetyl-L-carnitine for dementia).
Those people were already living with cognitive decline, and the doses were in grams. None of that describes a healthy adult taking a small capsule.
